Houston Parks and Recreation Centers
City Health Centers
City Multi Service Centers
Mail us your completed registration form or drop it off at our City sites.
Perform your wellness activities and check them off everyday.
At the end of 5 weeks, bring your completed card to our post-challenge celebration or to one of the City locations to receive your gift!
